Create a form
Start a new form (role) - add a title, description, and the questions candidates answer, then publish it live.
What it is
Creating a form opens the form editor, where you give the role a title and description and build the list of questions candidates answer. A new form starts as a Draft and only goes live to candidates once you Publish it.
There are three ways to start a form:
- New Form - start blank (this doc)
- Import - bring one in from a link or another tool (see Import a form or a list of candidates)
- Duplicate - copy an existing form from the home page's ··· menu (see Your forms list)
Steps (New Form)

On the Forms home page, click + New Form. A new draft opens in the editor.

Add the basics:
- (Optional) click Add logo to upload a company logo.
- Click the Title and type the role name.
- In the description area ("Add a description"), write the form description - it's a rich-text editor: format text, add lists and links, and type / to open a menu for inserting an Image or embedding a YouTube video (paste the video's link and click Embed).

Add the questions candidates answer. Click Add new question and choose a type:
- Short answer / Long answer - free text
- Number
- Multiple choice - List, Dropdown, or Multi-select
- Phone
- LinkedIn profile, or Other links - X (Twitter), Instagram, Github, YouTube, or any link
- Date
- File upload (e.g. résumé)
- Video recording
Name and Email questions are included by default and can't be removed. For each question you add, set its text, an optional helper description, mark it required if needed, and (for choice types) add the options.

(Optional) Open Form settings to configure: Verify email on submission, Notify about new submissions, Show on public page, and a Custom success message (what a candidate sees after applying). See Form settings and the careers-page docs for related settings.

(Optional) Click Preview to see the candidate's view, and Change theme to restyle the form. Theming is desktop only.

Click Publish to take the form live. It starts accepting applications and you're taken to its Responses page.
Note: Changes save automatically as you edit - there's no separate Save button. Publish is what turns a Draft into a live form; until then candidates can't see or apply to it.
Notes & limits
- A new form stays a Draft (not visible to candidates) until you Publish it.
- Name and Email are always present and can't be deleted.
- Images inserted into the description upload directly into it - accepted formats are JPEG, PNG, WEBP, and GIF, up to 10 MB each. Once an image is placed, drag a corner handle to resize it.
- An embedded YouTube video plays inline wherever the description is shown, including the public form candidates apply through.

- Candidate stages (New / Shortlisted / Interview / Archived…) are set up on the Responses page, not in the editor.
