SDE-2 — Backend / Fullstack Engineer

FOXO is a health and human services company that builds precision longevity systems tailored to individual biology and environmental factors. We’re hiring an SDE-2 to own core parts of the member journey end to end — from the APIs and data models behind onboarding and lab data to the logic that powers personalized health recommendations.

About the role

FOXO’s backend is where some of the most consequential work happens: biomarker results, personalized protocols, lab report parsing, and the business logic that connects health data to recommendations.
This is not a ticket-closing role. You’ll work under a Lead Backend Engineer, but the product surfaces you own are yours to design, build, improve, and support in production.
You’ll own the member-facing customer journey across backend systems, and if your profile is fullstack, you’ll also own selected web and internal tooling surfaces.
This is a full-time role for an engineer who wants meaningful ownership in a domain where correctness matters.

What you will build

Member onboarding and profile APIs — the data layer for health goals, conditions, supplement history, and wearable devices.
Lab report ingestion — receive parsed biomarker data from the extraction pipeline and normalize it into a member’s longitudinal health record.
Protocol delivery APIs — build the endpoints that serve personalized supplement and lifestyle recommendations with strong correctness and low latency.
Member progress and history — power longitudinal biomarker views, trend calculations, reference range evaluation, and alert thresholds used by clinicians and nutritionists.
Notification and communication systems — scheduled reminders, protocol check-ins, and biomarker follow-ups that need reliable delivery at scale.
If you’re fullstack, you’ll also build member-facing Next.js surfaces and internal ops tooling used by clinical and operations teams.

Tech stack

Python, FastAPI, Supabase (PostgreSQL), GCP Cloud Run, pgvector, GitHub Actions, and Claude API (Anthropic).
For fullstack work: Next.js, React, TypeScript, and Tailwind CSS.
We use Supabase Row Level Security heavily, so you should be comfortable reasoning about access control and writing policies correctly around sensitive member data.

What we’re looking for

3–5 years of backend engineering experience building APIs that real users depend on, including handling production issues when things break.
Strong Python fundamentals and a habit of writing clean, testable code.
FastAPI or similar async framework experience with a real understanding of async behavior at runtime.
Strong PostgreSQL skills across schema design, indexing, query optimization, and debugging slow queries.
REST API design experience including versioning, change management, and designing endpoints other engineers consume.
Frontend literacy is a plus — especially React or Next.js familiarity that helps in a small, fast-moving team.
For fullstack candidates: production-level comfort across Python/FastAPI and Next.js/React, strong TypeScript habits, and experience building internal tools or data-heavy interfaces.
Across both profiles: you write pragmatic tests, can own a feature from schema design through deployment, and understand the basics of handling sensitive health data responsibly.

Green flags

You’ve built a user-facing product journey such as onboarding, profile, or notifications and understand the full lifecycle.
You’ve worked in health, fintech, or another regulated-data domain where correctness is non-negotiable.
You’ve used an LLM API in production and understand token costs, reliability, and schema mismatches.
You ask why before you build, and want to understand the real member experience behind the systems you design.
